The Peer Advising Office distributes a number of forms to undergraduate students, available at the Peer Advising Office. This page is intended to help students determine if they need a form, and if so, what form they should complete and what process they should follow.
Independent Study Form
What is it used for?
To add an independent study course before the add/drop deadline. An independent study proposal must be attached. After the drop deadline, the Undergrad Late Add/Drop form is used.
Who needs to sign it?
Faculty sponsor
Note: Department approval is not needed
Where does it go?
After the form has been filled out and signed, the student brings it to the Department of Human Development Office, UDC-407 where the course will be added to the student's schedule.
Special notes/action:

The course will be added by the Registrar's Office after the late fee has been paid.
Internship Form
What is it used for?

Internship site supervisor and faculty sponsor.

Undergraduate Application for a Degree
Where can I pick one up?
Students can pick up a copy in the Peer Advising Office or print one from the Registrar's Web Page (http://registrar.binghamton.edu).
What is it used for?
To notify the registrar of a student's intention to graduate in a particular semester and to activate the formal degree-conferral process.
Who needs to sign it?
The student.
Where does it need to go?
After filling out the form, the student brings or mails it to the Office of the Registrar (SW-119).
Special notes/action:
- The declaration is valid for only one semester and must be filled out again if the student fails to meet all the curriculum requirements for that declaration.
- To graduate in a given semester, the form should be filed by the Drop Deadline (this is an anchor to info on the drop deadline) of the semester prior to the desired semester of graduation. (e.g. to graduate in spring 2008 the form must be filled out by fall 2007).
- Minors should be noted on the form.
- If you are a double-degree student you must complete two forms, one for each degree.

Undergraduate General Academic Petition
What is it used for?
- Any exception to Academic Policies, such as: Requesting a waiver of, substitution of, or exception to a major requirement. This includes requesting that a course is counted out of category, a change be made in the evaluation of your requirements, or taking more than eight credits of internship/independent study. Students must attach a copy of the current syllabus to the petition. Additionally students must clearly and concisely articulate the way in which the petitioned course falls within the parameters of the requested category change.
- Late change of grading option (both from letter grade to P/F or from P/F to letter grade).
- Change a student's major from a BA to BS or BS to BA
- Change a student's catalogue year
- Requesting a minor
Where does it go?
- After the student has obtained the appropriate signatures, the form is given to the undergraduate academic adviser for processing.
- A copy of the petition form will be mailed to the student once it has been processed. Students requesting a course waiver or substitution should be aware that the petition must be reviewed by the Academic Standards Committee. Check with the Peer Advising Office for the dates of the committee meetings.
Extension of Incomplete Grade
What is it used for?
To allow a student to retain a grade of incomplete for more than one semester after he or she has received it. Normally, an incomplete grade will become an F at the end of the semester following the one in which the student received the incomplete.
Who needs to sign it?
Student, the instructor of the course in which the incomplete was received and the CCPA undergraduate academic adviser.
Where does it go?
After the form is completed and signed by the instructor, it is brought to the Department of Human Development Office for approval by the academic adviser.
The student should return to the Department of Human Development Office within the week to reclaim the form. The student then takes the white and yellow copies to the Registrar's Office (SW-119) and keeps the gold copy for his or her file.
Late Add/Drop Form
What is it used for?
- to add or drop a course after the add or course withdraw deadline has passed
- to change a course to an option of audit
- to change the section of the same course after the drop deadline
- to change the listing of a cross-listed course from one department rubric to another.
Who needs to sign it?
Course instructor and the CCPA undergraduate academic adviser
Where does it go?
- Once the student has received all the above signatures, the form is taken to the Student Accounts Office (SW-117) if a late fee is to be paid.
- Once the late fee is paid, the student takes the petition to the Registrar's Office (SW-119) for processing. Petitions not received by the Registrar's Office within one week of the undergraduate academic adviser's dated approval will not be accepted.
- Students will receive the pink copy of the processed form in the mail.
Petition to Take Courses Elsewhere
What is it used for?
To request to take course(s) at another institution with the approval that the credits for the course(s) will be transferred into and applied toward the student's degree requirements. It is most often used for summer course work.
Who needs to sign it?
CCPA undergraduate academic adviser
Special notes/action:

Address Change Form
What is it used for?
To notify the Registrar's Office that a student's local residential or permanent address has changed. http://registrar.binghamton.edu
Who needs to sign it?
The student fills it out (no signatures required).
Where does it go?
Registrar's Office (SW-119).
Withdrawal Form
What is it used for?
To withdraw from Binghamton University. The student must withdraw from all courses.

Special notes/action:
This form is mandatory for undergraduates who wish to withdraw from an entire semester. The deadline for final withdrawal is the last day of classes in a given semester. This is important regarding any possible tuition refund eligibility during the acceptable refund periods.
Overload Request Form
What is it used for?
Students who want to enroll in more than 18 credits must complete an overload request form.
Who needs to sign it?
The Department of Human Development chair and the undergraduate academic adviser
Where does it go?
After the form has been signed by both the department chair and the academic adviser, the overload is processed into the system by the academic adviser.
Special notes/action:
Allow two weeks for an overload request to be processed.
Requests for overloads of between 21-24 credits will be reviewed by the Academic Standards Committee. Student should allow one (1) month for the request to be reviewed.
